Lazy bank/depository. I suspect a coin like that gets rejected from the coin sorting machines. But it is a pain in the neck to send to the mint for redemption. So, they slip it into a roll and pass it on to an unsuspecting pigeon.
I typically do this with PMD coins that aren't accepted by coin sorting machines. However, with the more severe cases, I simply pass them to a cashier when paying exact change.
