Grease filled die. Lets see some feed back from the error experts.
That is not a grease filled die. It is a great photo of machine doubling. Flat and shelf like.

Yes, MD as said above. That was not caused by a grease filled die.
Definitely machine doubling. Very common.
Is the C reverse special for some reason? It seems to match the A reverse in your diagram.
Mint mark appears as a die chip. Am I seeing that correctly?

I think it looks that way because the machine doubling smooshed it.
