As stated your coin is PMD (Post Mint Damage) Not A Mint Error.

What could have possibly been used to do such a thing?

Don't limit your thinking to a coin being handled easily in and out of pockets or purses. Treated with care as if it was made from fine Baccarat crystal.

How many have you seen on the ground in a parking lot; left in pockets going through laundry and ending up in the laundry machine stuck; throw in wishing wells, dropped into all sort of places; dropped from the tall buildings; bottom of purses with finger nail polish spilled on it; and an unlimited amount of other things.

Dimes make a great screwdriver at times.
