PMD... look closely and you'll see metal moved around the hole, making a Volcanic caldera. If this had happened during minting (struck through), the surface around the divot would be flat. Always look for moved metal...

May I trouble you to elaborate about the difference? Do you mean that there is some sort of "circular mound" around the hole (for PMD)? Thank you for the help and education.
Pretty much - the metal has to go somewhere. But unlike say sand it is a solid mass... whatever did the damage pushed it away...
