I'm wondering if anyone has ever seen a 1978 D quarter like this before. Any information would be appreciated
Welcome to Coin Talk. The quarter has environmental damage which occurred after it left the mint. This type of damage is generally called post mint damage. It looks like it was exposed to some sort of corrosive or sat out in the elements for a while. It is not a mint error.

Yes to Environmental Damage.. I have found many such as yours on the beach when Metal Detecting. Exposure to saltwater. Also could be the result of a Trolling accident.
