    It has a rim. It's just seen a lot of circulation wear, probably because too many people are searching penny rolls.

    Would a coin look like this if it were struck on a planchette that somehow missed the upset mill? Could such a thing actually happen?

    If the Obverse is raised then the Reverse has to be raised also.
    It happens at the same time when a blank witgh no raised rim is passed through the upsetting process which then becomes a Planchet.
