What causes this?

    Hey folks, This is my first post so please be gentle. I am pretty new at this, and wondered if y'all could tell me what caused this?

    So that glob next to date is normal?

    Not on a brandy new coin, but after 49 years of circulation, sure.

    The fresh gash on the head looks like it could have been made by a coin rolling machine. Last two photos are a bit blurry, did not see a glob next to the date.
    Everything I did see looks likes PMD.
