I may have done this one myself as a kid. I vaguely recall something about a vise and an ice pick and a hammer...

 

What happened to Sunday? Did I miss it?

 

I just ran across this one going through some junk to set aside...I don't remember when I got it or from where.

 

Maybe used as a button?

Here's a somewhat recently acquired piece.
Someone put some time and effort into making it what they wanted it to be.

 

Since the round holes are not clean through, I suspect that they held screws at one time. It may have been a base or an end piece for something that had two squares on it. What that might be I cannot even speculate.

 

Seems like it started as a bracelet piece (round holes) and then someone pounded a square nail through it twice. Why? Who knows?

 

I agree that 2 screws attached it to something and who knows what the 2 squares were 4. I've never seen anything like it.

 

Bracelet is a good guess. Not a necklace or button of some sort. The holes aren't right as you say.
