It's smooth mostly all around you can see where the thread maker went off track filled in D weighs 5.42gr
The reeding being missing from the edge of the coin is just damage after leaving the US mint. No way to know how it was done. The marks across the rims and fields are damage as well in my opinion. There is a thing called a coller clash but I don't think that's what this is. Maybe the reeding from a different coin was smashed into the coin.
We need to see the whole coin, both sides. They are called reeds, not threads, and the "thread maker" is the collar, and it's impossible for it to go "off track" in the way you seem to be thinking that it works. It might be a clipped planchet but nobody can say from only seeing parts of the coin. Edit: multiple threads about the same coin. From your other images it's just badly damaged.
Altered quarter. Not a mint error of any kind unfortunately. Here is some information on Reeded edge.. Reeding - Wikipedia https://share.google/YxRhjOJBLOb1fLvDJ
It’s post mint damage. The edge was ground down and the quarter was thrown in a vise with another coin.
