Might it also possibly be from being held by the edge in a device while heat was applied to the reverse? Just thinking outside the box since Reciprocally Deformed Convexo-Concavo Dies seem to be extremely rare.
Maybe sitting on a hot plate while being waterboarded. Drip water on top side only. Someone test it and report back.
