Thats one of the "errors".

 

It looks to be struck over / on another coin or token, but not sure what.
Photo causes size blindness.

 

Alright now down to the final part of the question. This coin was overstruck on a 2010 D presidential dollar. There are 4 of them

And here is the only real discernable design that could nail down which one it is.
Here are a couple new photos that show a little more of the overstruck details. Notice the eyes nose mouth and chin (center of Goerges face).

And on the reverse a little bit of Lady Liberty face arm and lower torch.

 

For me seem to be uncentered boadstrike.

Yes, here are some 3rd side photos.

 

I am leaning towards that one also.
Nose, beady eyes, and mouth position.
Still not sold on the chin.

There were 300 pieces done in 2012.
In collar, out of collar, small dollars to large dollars. Some of them had partial and full gold plating.
