2006 N Dakota Quarter Error?

    I ran across this 2006 N Dakota quarter with what appears to be a strike error with 3 rectangular punch marks on Washinton's face. Has anyone seen anything like this to explain how it would have happened? Would this be rare?

    Thanks. There is also another spot just like it on his chin. There are a few spots of corrosion on the coin. It looks like a strike through but at the same time it looks man made.

    Seems too clean to be manmade. No other damage to coin and to displace that material, it would typically bulge up around punch area or warp the coin
