What Happened to this 1945 S War Nickel?

I just found this curious nickel while searching a bank wrapped roll. At first I thought it was a hobo nickel, then some kind of severe environmental damage, then a struck thru, but now I have no idea. Could the dies themselves have been damaged?

That's more about reducing the size of a coin, but could be that it was hit with high voltage, or subjected to high heat.
Definitely PMD.

 

I was thinking it kinda looks like it was used at a "fuse"

 

It’s definitely damaged but it’s a keeper.

 

Smooth with some warping. A bit wider on one side.
