It would be helpful to know the denomination/date. I've seen this before; I think it happens because the dies are plated, and when plating chips off over time you get these raised areas.
Why, I can see the atoms apple also. Just kidding, but are you seeing something that is in process and is in early stage of it occurrence in split platting. if it is how long will something like that accrue until you see the splt.

So, what tipped ya'll off that this coin was a Jefferson nickel? It was his eyes wasn't it? For me it was the lips, thar purty.
Prior to 2004 yes. Now he faces right. The nickels produced for Lewis & Clark changed the face and the direction. But with a camera and a computer, you can make him face any direction.
That's not a Jefferson...no where on a Jefferson Nickel do you have the letters D, G and R (or I for you devil's advocate types) with dots between them..
Well @eddio said it was a 1999 nickel, so here is a picture of a pretty beat up Canadian nickel...My guess is eddio is messing with us or he likes all the die chips\gouges (or whatever they are) in and around the eye of the Beloved Queen.
I was really hoping I was going to get a response from the two I quoted. I guess my post conveyed I thought it was a Jefferson also with the "purty(pretty) lips" comment. I put an emoticon for the pretty lips comment, then the great another smart-a$$ comment emoticon, then the oh-boy no one is buying this emoticon, last but not least the wink in hey I know its not a Jefferson nickel emoticon. I'll try to use more emoticons in my post to convey my message better.
