    Could be, I was thinking more like a vise.

    Doubt it, there's uneven pressure, which would allude to an off-centered blow from a hammer. A vice would have a much more even imprint.

    I don't see how a hammer could hit both sides at the same time, or one side at a time without flattening the other.

    My vice can be "mis"aligned to hold Items with non-parallel sides but the grip pattern on the jaws is different. Still, i'll go with a vice or other gripping device being the culprit here.

    Could this be a partially destructed coin?
    I read somewhere that waffling is the first step in coin destruction before they are shipped off to be separated into their base metals?

    No, the waffling machine they are using is totally different
