A quarter is 1.843 times the area of a dime. A quarter sized blank cut from dime stock would weigh 1.843 times a dime's weight of 2.27g, or about 4.18g. I believe yours is, indeed, a quarter stamped on dime stock. The ring and "reeding" on the obverse are indicative of damage from a coin roller. Just guessing, I would say this was an extra coin in the end of a roll and the resulting extra pressure of the machinery doing the rolling caused the marks.
That is pretty much what I figured about the circle around Washington's head. I thought that it was caused by a coin rolling machine, and it appears to be cut into the coin. The "rays" around the edge that extend from the reed edge are raised and appear to have been made when the coin was pressed. I have no idea what could have caused this. I am really disappointed in NGC. They did not take into account the weight of the coin or the thickness when they body bagged my coin and labeled it damaged to resemble an error.
