1968 D-quarter error ?? help please

i just open an 1968 mint envelope with 12 coins,didn't open the plastic seal

I have a 1968-D quarter uncirculated , there is no D printed on front or back.
is it common?

It's possible that two Philly quarters were placed in the set by mistake. This is not considered an error.
